| 00:28 | Access our Annual Survey flyer template using Canva | Click on the “Create Custom Survey Flyer” button at the top of this page and login or create a free Canva account to access the template. |
| 00:43 | Copy your unique survey link that we’ve sent via email | Locate the “Fitness Passport Annual Survey 2025” email sent from [email protected] dated 8/05/2025. Scroll down to Step 2 in the email and locate your unique survey link in blue text. Highlight each link completely and copy it. |
| 01:08 | Embed your unique link in the survey flyer template | Find the button towards the bottom right that says “Complete Survey” and hover over it. Click on the three dots for more and locate the link button. Paste your unique survey link in the first textbox and click “Done”. |
| 01:49 | Generate a QR code using your unique survey link/s | Locate the “Apps” button on the left side panel and click on it. In the search field at the top, type “Dynamic QR Codes” and click the first option with the blue icon. Next, select the 6th design from the top. In the link textbox, paste your unique survey link. Change the pattern colour and eye colour to our FP blue colour (#0093D3) and then click “Add to design”. Finally, use the corners of the QR code to scale the QR code and fit it in the white rounded square box on the right side of the flyer. If you wish to use our virtual background design for virtual meetings and wallpapers, copy the QR code and paste it on the virtual background template, scaling the QR code into the white rounded square box. |
| 04:48 | Export the flyer as a PDF file | Flyer – Click the “Share” button on the top right, then click download. Select the file type as “PDF Standard”, then select either Page 1 or Page 2 as your design (depending on whether you want to promote the competition element of the survey or not). Finally, click “Download” and save your design to your folder. Virtual Background – Click the “Share” button on the top right, then click download. Select the file type as “PNG”, then select either Page 3 or Page 4 as your design (depending on whether you want to promote the competition element of the survey or not). Finally, click “Download” and save your design to your folder. |
