Fitness Passport Pty Ltd – Collection Notice 

Fitness Passport Pty Ltd (ABN 50 118 426 029) (“Fitness Passport”, “we”, “us”, “our”) is committed to protecting your privacy. This Collection Notice explains how and why we collect your personal information when you complete this survey.

2. Facts and Circumstances of Collection 

We are collecting your personal information directly from you through this online survey, hosted on SurveyMonkey. The survey is being conducted on behalf of your employer to gauge interest in a workplace fitness program. Completion of this survey is voluntary. 

The personal information collected through this survey includes: 

  • Full name (optional) 
  • Email address (required) 

Please note: This survey is hosted and processed by SurveyMonkey, which is based in the United States of America. Your survey responses will be transmitted to and stored on SurveyMonkey’s servers in the USA. Details of how SurveyMonkey manages personal data are available at SurveyMonkey’s Privacy Policy. 

3. Is Collection Required or Authorised by Law? 

Collection of your personal information through this survey is not required by law. It is voluntary. Fitness Passport’s handling of your personal information is regulated by the Privacy Act 1988 (Cth) and the Australian Privacy Principles. 

4. Purposes of Collection 

We collect your personal information for the following purposes: 

  • To understand interest in, and preferences for, a workplace fitness program at your employer’s organisation; 
  • To contact you with a program sign-up link once negotiations with fitness facilities are finalised, if you have provided your email address; 
  • To aggregate and analyse survey responses to tailor the program offering to your workplace (responses will be collated and used to negotiate with gyms and fitness facilities in your area); 
  • To administer and improve Fitness Passport’s services and programs. 

5. Consequences if Personal Information is Not Provided 

If you choose not to provide your email address: 

  • We will be unable to send you the program sign-up link when the workplace fitness program becomes available at your organisation. 
  • You may still participate in the program through your employer’s communications, but you will not receive a direct notification from Fitness Passport. 

If you choose not to complete the survey at all, Fitness Passport may not be able to assess the level of interest or specific fitness preferences at your organisation when negotiating with facilities. 

6. How We Usually Disclose Your Personal Information 

Fitness Passport may disclose your personal information to: 

  • Your employer: Aggregated and de-identified survey results may be shared with your employer to assist in program design. We do not disclose individually identifiable survey responses to your employer without your consent. 
  • SurveyMonkey Inc (USA): As our survey platform provider, SurveyMonkey processes and stores your survey responses on our behalf. See Section 7 below for further detail. 
  • IT and technology service providers: We use third-party technology vendors to operate our business and manage data, who may access personal information in the course of providing those services. 
  • Fitness facilities: Where you later sign up to the Fitness Passport program, your personal information will be shared with partner gyms, pools and fitness facilities to facilitate your membership access. 
  • Regulatory or government bodies: Where required or authorised by law. 

7. Overseas Disclosure of Personal Information 

Fitness Passport is likely to disclose your personal information to overseas recipients. Specifically: 

  • United States of America: Survey responses are transferred to and stored on servers operated by SurveyMonkey Inc, which is headquartered in the USA. 
  • Other jurisdictions: Fitness Passport may also use other technology or cloud service providers whose servers are located outside Australia, including potentially in the European Union, United Kingdom, or other countries

Where we disclose your personal information to overseas recipients, we take reasonable steps to ensure those recipients handle your information in a manner consistent with the Australian Privacy Principles, including through contractual arrangements. However, you should be aware that overseas recipients may be subject to foreign laws that could require disclosure of your personal information.
